This quick form provides us with important background information so we can fully prepare for your appointment.

Gather Your Tax Documents
Please collect and upload the following documents (as applicable):

Valid photo ID (Driver’s License or State ID)

Social Security cards (for you, spouse, and dependents)

W-2s from all employers

1099s (freelance income, unemployment, retirement, dividends, etc.)

1098s (mortgage interest, tuition statements)

Prior-year tax return (if not previously filed with us)

Income and expense summary for self-employment or small business

Childcare expenses (including provider’s EIN or SSN)

Health insurance form (1095-A, if applicable)

Bank account details for direct deposit

Upload Your Documents Securely
Once gathered, upload your documents to our encrypted Client Portal.

Attend Your Virtual Appointment
We’ll meet via video to review your documents, ask clarifying questions, and walk you through the next steps.

Review and Approve Your Return
After your return is prepared, you’ll receive a copy to review. Once you approve it, we’ll e-file your return securely on your behalf.
